refactor: extract ContextMenuHandler from main_window.py
Move _on_context_menu, _on_multi_context_menu, _is_child_of_menu into
gui/context_menus.py. Pure dispatch to already-extracted controllers.
main_window.py: 1400 -> 1140 lines.
